Life can be turned upside down after an accident or injury. For injuries
serious enough to require hospitalization, the effects are even more
significant. Not only is the patient's life "derailed"
by the injury, their family and friends are also affected. Rehabilitation
in Motion provides assistance to patients and family when planning for
discharge from hospital. We perform home assessments to evaluate safety
and determine home support needs and equipment requirements. (i.e. bathing, sleeping, cooking
etc). If necessary, arrangements for cleaning, meal preparation and
transportation can be made.
Most individuals are anxious to get back to their homes in the most
timely fashion possible. However, after injury or illness, one's
ability to function within the home may be restricted. The struggle
to regain independence in activities of daily living (ADLs) can be made
easier if help is provided for a period of time. Rehabilitation in Motion
can attend your home to discuss your needs and match these with community
resources. A plan will be formulated and coordinated with your physician,
the funder and various service providers, depending on your specific
needs.
